How to Print Risk Input Forms

From the Risk Assessments Home screen, you can choose to print any of the available Risk and Disease Analysis Input Forms for your use in gathering and recording data from the Patient. This is helpful if you do not have a computer in the surgery for PreViser data entry, and need to take the information to another computer for the risk assessment.

Note that all printed Input Forms will include the information you have in your default settings on the Options page, in the "Reports Prepared By" section (your name, etc.).

You have two options in printing and using these forms:

1. You may print completely blank forms (from the Home page).
2. You may print a form partially completed with information you have started to enter for a specific patient's Risk Assessment.

 

To print a completely blank form:

1. To print a blank form from the software, start on the Risk Assessments Home screen, shown below:

2. For a Perio or Oral Cancer Risk Assessment, to print a form suitable for all ages, click Print Perio Risk and Disease Forms or Print Oral Cancer Risk Forms.

A Clinical Data Form for Perio or for Oral Cancer will print on your local printer.

3. For a Caries Risk Assessment, there is an additional step after you click Print Caries Risk Forms.

The Print Caries Input Form screen displays, with a menu of various Caries forms available, depending on the age of the patient, as shown below:

4. Click on the age-appropriate form you wish to print. A Caries Input Form will print on your local printer.

After you have printed the forms you want, click the Return Home button to display the Risk Assessments Home screen, or click any of the other options on this screen, as described in the Risk Assessment Home documentation.

To print a partially completed form:

After beginning a Risk Assessment, you can print the appropriate input form directly from the input screen. Simply click the "Print Form" link in the top right corner of any page of a Risk Assessment. A form will print, pre-filled with whatever data you have already entered and saved.

Note that partially completed forms print with each patient's identifying data (name, etc.) so you will need to handle these forms in a way that protects the patient's privacy.
