Treatment Option Plan Reports

A Report, or Treatment Option Plan, is a visual presentation of the information gathered and calculated from either of the three types of Risk Assessment (either Perio, Caries, or Oral Cancer). Each type of Risk Assessment produces a report that may be viewed in either one or two design formats, including the same information but designed for two different purposes:
  • A Treatment Option Plan in Clinical Report format is a more concise summary meant to store in the patient’s file in your practice’s records, or shared with other clinicians.
  • A Treatment Option Plan in Patient Report Format is a patient-oriented report intended to be printed and given to the patient.
The following is a list of all the possible report formats. (There are a variety of Caries reports targeted to a specific age range as indicated. Note that currently, Oral Cancer Risk Assessments only produce a report in Patient Format.)
  • Perio Clinical

  • Caries Clinical (1-4)
  • Caries Patient (1-4)
  • Caries Clinical (5-8)
  • Caries Patient (5-8)
  • Caries Clinical (9-18)
  • Caries Patient (9-18)
  • Caries Clinical (19-33)
  • Caries Patient (19-33)
  • Caries Clinical (34 and up)
  • Caries Patient (34 and up)
  • Oral Cancer Patient

Sound complicated? Basically, there are three types of Risk Assessment: Perio, Caries, and Oral Cancer. Each of these types produces a report that can be viewed in either one or two report formats: Patient or Clinical.
